Several conditions that can lead to hypothermia are:
improper clothing and equipment
wetness
fatigue, exhaustion
dehydration
poor food intake
alcohol intake

If your body loses heat faster than it can be replaced, then your body begins to cool and the risk of hypothermia rises.

Ways to protect yourself:

1.) Be aware of the current weather situation before you begin.

2. Proper clothing is the most important element in protecting yourself while exercising in the cold.
a) Always wear a hat
b) Wear a neck gaiter or balaclava to protect your face and neck
c) Wear thicker socks, preferably ones designed to draw moisture away from the skin
d) Wear gloves or mittens
e) Dress in layers, but don't overdo it. Wear one less layer of clothing during exercise than you would if you weren't exercising. It's okay to be a little chilly at first.

3. Stay hydrated.

4. Don't forget an adequate warm-up and cool-down to help prevent injury.

5. Be sensible. If it's too cold, workout indoors.
